World TB Day 24th MARCH

The 2014 World TB Day campaign 24th MARCH  is focused on individuals around the world who have found new ways to stop TB and can serve as an inspiration to others. The idea is to recognize people who have introduced a variety of innovations in a variety of settings:
  • research aimed at developing new diagnostics, drugs or vaccines
  • operational research, aimed at making TB care more effective and efficient
  • new approaches to helping people gain access to TB diagnosis and treatment
  • novel partnerships between actors in the fight against TB
  • advances in integrating TB care into health systems
  • new approaches to providing support from members of the community to people affected by TB
  • innovative ways of raising awareness about TB.

Causes, incidence, and risk factors

Pulmonary tuberculosis (TB) is caused by the bacteria Mycobacterium tuberculosis (M. tuberculosis). You can get TB by breathing in air droplets from a cough or sneeze of an infected person. This is called primary TB.
Most people who develop symptoms of a TB infection first became infected in the past. However, in some cases, the disease may become active within weeks after the primary infection.
The following people are at higher risk for active TB:
  • Elderly
  • Infants
  • People with weakened immune systems, for example due to AIDSchemotherapy, diabetes, or certain medications
Your risk of contracting TB increases if you:
  • Are in frequent contact with people who have TB
  • Have poor nutrition
  • Live in crowded or unsanitary living conditions
The following factors may increase the rate of TB infection in a population:
  • Increase in HIV infections
  • Increase in number of homeless people (poor environment and nutrition)
  • The appearance of drug-resistant strains of TB
In the United States, there are approximately 10 cases of TB per 100,000 people. However, rates vary dramatically by area of residence and socioeconomic status.

Symptoms

The primary stage of TB usually doesn't cause symptoms. When symptoms of pulmonary TB occur, they may include:
  • Cough (usually cough up mucus)
  • Coughing up blood
  • Excessive sweating, especially at night
  • Fatigue
  • Fever
  • Unintentional weight loss
Other symptoms that may occur with this disease:
  • Breathing difficulty
  • Chest pain
  • Wheezing

Signs and tests

Examination may show:
  • Clubbing of the fingers or toes (in people with advanced disease)
  • Enlarged or tender lymph nodes in the neck or other areas
  • Fluid around a lung (pleural effusion)
  • Unusual breath sounds (crackles)

Treatment

The goal of treatment is to cure the infection with drugs that fight the TB bacteria. Treatment of active pulmonary TB will always involve a combination of many drugs (usually four drugs). All of the drugs are continued until lab tests show which medicines work best.
The most commonly used drugs include:
Other drugs that may be used to treat TB include:
  • Amikacin
  • Ethionamide
  • Moxifloxacin
  • Para-aminosalicylic acid
  • Streptomycin
You may need to take many different pills at different times of the day for 6 months or longer. It is very important that you take the pills the way your health care provider instructed.
When people do not take their TB medications as recommended, the infection becomes much more difficult to treat. The TB bacteria may become resistant to treatment, and sometimes, the drugs no longer help treat the infection.
When there is a concern that a patient may not take all the medication as directed, a health care provider may need to watch the person take the prescribed drugs. This is called directly observed therapy. In this case, drugs may be given 2 or 3 times per week, as prescribed by a doctor.
You may need to be admitted to a hospital for 2 - 4 weeks to avoid spreading the disease to others until you are no longer contagious.
Your doctor or nurse is required by law to report your TB illness to the local health department. Your health care team will be sure that you receive the best care for your TB.

Support Groups

You can ease the stress of illness by joining a support group where members share common experiences and problems.

Expectations (prognosis)

Symptoms often improve in 2 - 3 weeks. A chest x-ray will not show this improvement until later. The outlook is excellent if pulmonary TB is diagnosed early and treatment is begun quickly.

Complications

Pulmonary TB can cause permanent lung damage if not treated early.
Medicines used to treat TB may cause side effects, including liver problems. Other side effects include:
  • Changes in vision
  • Orange- or brown-colored tears and urine
  • Rash
A vision test may be done before treatment so your doctor can monitor any changes in your eyes' health over time.

Calling your health care provider

Call your health care provider if:
  • You have been exposed to TB
  • You develop symptoms of TB
  • Your symptoms continue despite treatment
  • New symptoms develop

Prevention

TB is a preventable disease, even in those who have been exposed to an infected person. Skin testing (PPD) for TB is used in high risk populations or in people who may have been exposed to TB, such as health care workers.
A positive skin test indicates TB exposure and an inactive infection. Discuss preventive therapy with your doctor. People who have been exposed to TB should be skin tested immediately and have a follow-up test at a later date, if the first test is negative.
Prompt treatment is extremely important in controlling the spread of TB from those who have active TB disease to those who have never been infected with TB.
Some countries with a high incidence of TB give people a BCG vaccination to prevent TB. However, the effectiveness of this vaccine is controversial and it is not routinely used in the United States.
People who have had BCG may still be skin tested for TB. Discuss the test results (if positive) with your doctor.

विवेकानंद बोले "आज से मैं

क्या ये हर युग में सम्भव नहीं है ? आज का  युग  पाशविक  वृतियों से भरा पड़ा है। 

संस्कार धीरे धीरे लुप्त होते जा रहे है ।  आधुनिक विचारधारा बहोत जरुरी है पर संस्कारो से वंचित होना यानि के एक तरह से अपने वंश को नावंश  करने जैसा है । विवेकानंद जैसे कई  लोग आज भी हमारे बिच मौजूद है। पर हम  में से कई लोग इन बातो पर हस्ते है और सोचते है कि ये सब बकवास है ।

 अगर जरा सा गौर करे तो.अपनी बेटी या बहिन कि शादी करेगे तो सुशिल पुरुष मर्यादा वाले पुरुष के साथ और घर में भी संस्कार चाहेगे तो वो भी शुद्ध!!!!!पर बाहर  इतना  दिखावा क्यों ?

 आज के युगं में किसी भी प्रतिभा को आगे आने के लिए,हम  अक्सर हर एक फील्ड में यही देखते है कि अगर औरत है तो उसे अपने चारित्र्य  को दाव पे लगाना पड़ता है और अगर पुरुष है तो उसे पैसे रुपयो से अपने जमीर को बेचना पड़ता है । ये सब हमारे में से ही कुछ लोगो कि सोच है।  . . . . . .  जो इन बातो पे गौर नहीं करते है पर,भूल जाते है  कि कल उनको किसी न किसी स्वरुप में इन सब का बड़ा भरी भूगतान  देना पड़ेगा ।  जो कुदरत का कानून अटल है ।  देश विदेश में जो अपने असली मूल और संस्कार को कायम रख कर  , आगे बढ़ता है वो इंसान मिसाले तारीफ  है।  हमारे सामने ही कई  ऐसे उदाहरण आम जिंदगी  में भरे पड़े है ।

पढाई लिखाई करने से इंसान कि सोच खुलती है विचार ऊँचे होते है,पर आज के युग में लोग देखा देखि में ही असलियत से मुह मोड़ रहे है। हालाँकि  सब कुछ जानते है हैम पर उस बात पर विचार नहीं करते है ।  अगर हमारे बच्चे या किसी एक बच्चे को या किसी  इंसान को अगर हम  कुछ उदहारण द्वारा उसके जीवन को मोड़ सके तो उनकी जीवनको हैम नयी दिशा और नया जीवन प्रदान कर सकते है। 

पर उसके पहले हमें खुद के अंदर झांकना पड़ेगा कि ,  हमारे भविष्य  के रचैता हैम खुद हमारे कर्मो से ही है।

Tips for safe and eco-friendly Holi 

 Celebration



 Holi, a festival of colours, fun, joy and harmony will be celebrated this year on�7�th March. Over a period of time, a few unhealthy practices of using artificial colours and consuming �bhang� and �alcohol� have creeped in. The dream of an eco-friendly and non- hazardous Holi can be true only when every individual who participates in this festival, makes efforts from his/her side to protect the environment and to keep away from practices that spoils and harm�environment and local ecosystem.
Judicious Use Of Water:�In the current situation, when most cities in India are facing acute water scarcity, the wasteful use of water during Holi, is also being questioned. Thus recommend to play dry Holi with Gulal only.This will also save the risk of falling ill.
Avoid synthetic colours:�Prefer preparing eco-friendly natural colours at home; haldi concoction for yellow, beetroot concoction for red, and methi or neem concoction for green. In any case, organic colors are now easily available in the market. 
Smart use of Bonfire:Lighting of wooden pallets for the bonfire is a serious environmental issue. Would request you to lit one symbolic community bonfire, rather than burning individual bonfires across your locality as a way to reduce wood consumption. The other way is to lit bonfires by using waste material rather than wood.
Protect your eyes from attacks:Eyes are extremely vulnerable on Holi because of the use of harmful chemicals in colours these days. The chemicals can badly infect your eyes due to the toxic chemicals mixed with colours and can even cause blindness.
Avoid using plastic bags and balloons

Lot of people have a habit of throwing small plastic bags and balloons filled with water on each other. The remains of which create environmental pollution. Hitting each other with plastic bags and balloons during�Holi�can cause harm to some external organs. You should also stop other people from doing this.

Play Safe:�Don�t� play with someone if he/she is not willing to play and do not let others to be rude with you. The best option would be to play with natural colours.

Preparations before Holi Celebration:

Make sure that you apply ample quantity of cream on your face before and after playing with colours. Cover up the body to the maximum with synthetic garments so that they do not soak much water.
Oil your hair well, it will make it easy for you to get rid of the colour stuck in the hairs. Also it will limit the effect of chemicals to hairs.
Keep your lips tightly locked so that colours do not enter your mouth. Also protect your face from any possible attack on face.
Try to avoid going outside of your premises in the peak hours of the festival. This way you can prevent yourself from being engaged with rough friends.
During travel, keep the window of your car tightly closed, even if you dont have an AC car. Stay at a safe distance from the mob celebrating holi.
Take a bath with luke warm water after the Holi celebration is over. Avoid frequent washing of face and regular baths during the Holi celebration. It will ruin your skin with dryness.
Do not run and jump on wet floors as you may slip and pose the danger of bone fracture.
Avoid drinking of bhang and alcohol during festival. Do not drive when you are high on alcohol.
If you are asthmatic, take preventive doses in advance and keep S.O.S. inhalers [e.g. Astheline] handy. For such people, it is best to avoid playing Holi, if possible.
Keep anti-oxidants and other anti-allergic medicines ready for any accident or if you encounter any skin problem.
